Your Rights

You have the right to:

  1. Get a Copy of Your Medical Record
    You can ask to see or get a copy of your medical record and other health information. We will provide a copy or summary within 30 days of your request. A reasonable, cost-based fee may apply.

  2. Request a Correction
    You can ask us to correct health information that you believe is incorrect or incomplete. We may deny your request, but we will tell you why in writing within 60 days.

  3. Request Confidential Communications
    You can ask us to contact you in a specific way (for example, at work or by mail) or to send mail to a different address.

  4. Request Restrictions
    You can ask us not to use or share certain health information for treatment, payment, or healthcare operations. While we are not required to agree to all requests, we will consider them carefully.

  5. Get a List of Disclosures
    You can ask for a list (accounting) of the times we’ve shared your health information for six years prior to the date you ask, and who we shared it with.

  6. Choose Someone to Act for You
    If you have given someone medical power of attorney or if someone is your legal guardian, that person can exercise your rights and make choices about your health information.

  7. File a Complaint
    If you feel your privacy rights have been violated, you may file a complaint with us or with the U.S. Department of Health and Human Services (HHS).

Our Uses and Disclosures

    Example: A doctor sharing test results with a specialist for consultation.
